Description:
Job Summary:
The Care Coordination Social Worker Sr. (SW Sr ) is responsible for assessing transitional care needs, coordinating care across the continuum, and engaging with patients and families to meet care needs. As an expert resource for complex situations, the SW Sr collaborates with the care team members on patients' psychosocial and resource needs, ensuring timely and safe discharge planning. The role integrates patients' transitional care plans into individualized discharge plans based on needs and resources.

Specific functions within this role include:

Care Coordination:
Assesses transitional care needs and coordinates care for complex patients.
Provides guidance on community-based service offerings and resources to assist patients/families.
Serves as a specialist on issues related to psychosocial and discharge needs, end of life care planning, and resource needs.
Partners with the care team to assess and implement timely discharge plans.

Complex Disposition Planning:
Implements discharge planning for complex patients and identifies barriers for timely dispositions.
Understands eligibility criteria for resources to facilitate safe transitions.
Advocates for patients and empowers them in making autonomous healthcare decisions.
Ensures cultural and religious beliefs are considered in care planning.

Assessment:
Initiates assessments of patients' psychosocial risk factors and resource availability for discharge planning.
Collaborates with patients, families, physicians, and the care team to establish care progression and discharge plans.

Documentation:
Completes psychosocial assessments and maintains accurate documentation in medical records.
Tracks and documents all services arranged/delivered for patients.

Precepting/Mentoring:
Supports leadership in precepting new hires and mentoring less senior employees.
Serves as a preceptor and mentor for student interns in addressing challenging patient/family situations.

Professional Development and Initiative:
Completes all required professional competencies and education.
Participates in the development of protocols, procedures, and performance improvement initiatives.

Performs other duties as assigned.

Required Minimum Education:
Master's degree in Social Work from an accredited college or university.

Required Minimum License(s) and Certification(s):
LMSW in State of GA or LCSW.
BLS certification.
Certified Case Manager (Preferred) or Accredited Case Manager (Preferred).

Required Minimum Experience:
Minimum 2 years of experience in healthcare, acute care, skilled care, or related field.

Required Minimum Skills:
Excellent written and verbal communication skills.
Self-directed with the ability to function well under stress in a fast-paced environment.
Strong assessment, interview, organizational, and problem-solving skills.
Knowledge of local, state, and federal regulations and community resources.
Ability to collaborate effectively with the care team, physicians, and patients/families.
