**Job Specialty:** Floor Nurse - Clinic
**Job Duration:** 13 weeks
**Shift:** Monday - Friday, Primary Care
**Guaranteed Hours:** 40 hours per week
**Experience:**
- At least 1 year of general nursing experience with 6 months as a clinical nurse in an outpatient clinic, OR at least 2 years of general nursing experience with a minimum of 1 year in an ambulatory setting or case management experience.
- Previous case management experience is required.
**License:** Current full, unrestricted Registered Nursing license in Alaska
**Certifications:**
- Basic Life Support certification preferred.
- National Case Management certification preferred.
**Must-Have:** Proof of flu vaccine required
**Job Description:**
- Responsible for active management of patients, collaborating with medical providers and interdisciplinary team members to establish individualized medical goals.
- Coordinate multi-disciplinary care conferences for high-risk or complex patients referred to the case management process.
- Manage panel acute and chronic care needs and health maintenance, using electronic health records for reminders and outreach to patients.
- Participate in case management reviews for high-need patients with a multi-disciplinary team.
- Advocate for patients and their families, fostering decision-making, independence, and self-care.
- Use problem-solving and anticipatory skills to meet patient and provider needs promptly and efficiently.
- Facilitate coordination of patient referrals, track progress, and serve as a link for other case managers.
- Perform staff nursing duties, including assessment, documentation, education, and medication administration.
- Maintain competencies in safety and infection control, and ensure a clean, safe working environment.
- Work collaboratively with team members and participate in clinic team improvement efforts.
- Lead in the clinical team, participating in continued development of the case management role in primary care.
- Participate in triage roles and respond to emergency situations as required.
- Facilitate coordination of specialty clinic referrals, manage waitlists, and ensure completion of diagnostics.
- Act as primary nurse for specialty clinics, providing clinical support and preparing clinical space for procedures.
- Assist medical staff with educational in-services provided by specialty clinic providers.
- Responsible for intake, tracking, education, and outreach for OB patients, ensuring comprehensive care.
- Communicate and coordinate video teleconferencing visits and manage exercise treadmill test referrals.
- Utilize excellent assessment, communication, and multitasking skills in a fast-paced clinical setting.
