Requirements:

  • Required Certifications: State licensed
  • Required Skills: Emergency Department,Biopsychosocial assessment,Collect and analyze patient information,Conduct initial evaluation,Develop plan of care to meet patient/caregiver needs,Development of problem list,Mini-mental state examination,Assists patients with navigating the Department of Children and Families (DCF) process,Collaborative care,Coordinates with internal and external team members,Documents pertinent information to facilitate care coordination,Participates in care conference activities,Refers to rehab and long term care facilities,Documentation according to plan of care,Documentation of care delivery, plan, goals, etc,Documents patient response,Facilitate discharge and contingency planning,Individual counseling,Provide crisis intervention,Educates patient/caregiver to facilitate discharge,Provide / reinforce individualized patient/caregiver education,Emergency detention,Suicide/homicide/self-harm,High risk intervention patient vulnerability or safety risk,Knowledge of cultural issues,Care coordination,Discharge Planning,Plan of Care

Job Details:

  • Pay: $1814/week - $2043/week
  • Assignment Length: 13-week assignment
  • Shift: 4, 10-Hour 21:30 - 08:00
  • At least one year of experience required
