Seeking a caring, energetic, optimistic, and reliable Licensed Practical Nurse to join an organization that build's a world in which individuals

Job Description:

+ Conduct patient history intake and preparation

+ Collect and manage lab data

+ Conduct advanced telephone triage functions and participate in research activities

+ Act as a liaison between patients, families, and other healthcare professionals

+ Maintain accurate records and documentation

+ Monitor and order supplies

+ Perform quality assurance (QA) functions as directed

+ Assist with the orientation and training of new personnel and clinical staff

Qualifications:

+ State of MA LPN License

+ Associate's degree with LPN certification and 1-2 years related experience.

+ One year of supervisory or human service experience is desired.

+ Knowledge of and respect for diverse populations.

+ Must be able to travel within the region to access residential settings, employers, and community organizations.

Schedule: Monday-Friday 8:30am-3:30pm. NO WEEKENDS
