Rady Children's Hospital - San Diego has implemented an innovative, new Primary Care Mental Health Integration Program (PCMHI) through the Transforming Mental Health (TMH) initiative. This network of services provides accessible mental health services within the primary care medical home, and at our regional mental health specialty hubs across San Diego and Southern Riverside County. Our program is focused on supporting early identification, health promotion and illness prevention, in collaboration with our primary care pediatric partners. We have a growing team of clinicians focused on providing total child health and mental wellness.

As part of the PCMHI team, the Integrated Health Therapist "IHT" serves as a valued provider and partner in total child well-being, based in the primary care pediatrician's office and/or at an outpatient "Hub" location. The IHT will conduct a brief model of mental health services, including diagnostic assessment, triage, psychotherapy, and multidisciplinary team treatment planning. As a generalist provider, the IHT will offer education, connection to resources, counseling, problem solving, brief therapy, and crisis intervention. The IHT will develop integrated care plans utilizing knowledge of mental health, age, developmental stages, and culture. This role works closely with physicians to provide holistic care, monitor patient progress, and achieve treatment goals to improve both mental and physical health.

The IHT may engage in teaching opportunities including workshops or presentations for youth, families, providers, and/or community members on topics related to mental health, integrated care, and wellness. As TMH programming evolves, opportunities for collaboration in research projects may become available, as well as opportunities for professional advancement. A licensed IHT may provide supervision to pre licensed clinicians at the discretion of the Supervisor and as allowed by the academic institution and licensing boards; may also assist with administrative duties, as assigned by the Supervisor.

JOB SUMMARY:

As part of the MHI team , the Integrated Health Therapist (IHT) serves as a provider and partner in total child wellbeing, based in a PCP's office, at a regional outpatient Hub, or a combination of the two based on program needs. The IHT conducts mental health services, including diagnostic assessment, triage, psychotherapy, and multidisciplinary team treatment planning in the provision of clinical intervention. The IHT applies principles of social work, psychology, counseling, and other behavioral health practices to offer services such as brief therapy, psychoeducation, case management, problem solving, advocacy, and crisis intervention.

The IHT is connected to a patient/family primarily via referrals from the patient's PCP, which is facilitated by a Warm Hand-Off from the medical provider and followed by a same day assessment and triage by the IHT to identify and address mental health needs early. Post-assessment, the IHT provides brief, evidence-based therapy, and coordinates referrals, where appropriate.

The IHT comanages patient care along the PCP and acts as the main contact for and liaison between the patient's referring provider, psychiatry team, care coordination team, and other treatment team members.

The IHT supports medical providers through ongoing consultation and provides continuous mental and behavioral health education. Patient care is provided both in person and via telehealth depending on patient preferences and scheduling availability.

MINIMUM QUALIFICATIONS:

Master's Degree in Social Work, Marriage and Family Therapy, Counseling, Psychology, or related field

2 years of experience

CA Driver's License and proof of auto insurance

Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) in the State of California

Successful completion of Rady Children's Medical Staff credentialing process

Knowledge of a range of tools, methods and treatments for a wide range of mental health issues and mental health disorders; ability to treat, study and prevent mental health issues with appropriate therapies

Knowledge of theories and methodologies for brief, goal- and solutions-focused, proactive interventions, psychological counseling, ability to conduct a planned counseling interaction and design a counseling process to facilitate patient's progress toward meeting longitudinal and shared care plan goals and objectives

Experience in assessment and treatment of children and adolescents

Knowledge and expertise in emotional, behavioral, developmental and psychology norms, pathology, differential diagnosis, and evidence-based treatment options and comfort in using a generalist approach with youth of all ages: - Infant and Preschool (0-5 years); - Child and Adolescent (6-17 years); and - Transitional aged youth (18-26 years).

Knowledge and expertise in child development, family therapy, and parent management training

Extensive knowledge of safety planning and risk assessment

Knowledge and strong ability to perform same-day triage and assessment

Understanding and/or interest in the differences with traditional mental health care: Through integrated care, more time is spent in direct patient care, and providers operate at a more accelerated pace in a high patient volume setting, working flexibly in a sometimes unpredictable schedule to be available for Warm Hand-Offs during medical provider visits

Enjoy working on a multidisciplinary team, and understand the importance of shared care plans in order to deliver holistic, high-quality care to pediatric patients

PREFERRED QUALIFICATIONS:

4 years of experience

Previous experience in integrated mental health care settings

Knowledge and expertise in school and individualized education plans/504 plans, child abuse and child welfare

Experience in trauma, bereavement, chronic illness, catastrophic diagnosis, family violence, child protection, and bioethics issues

Knowledge of evidence-based practice utilized in clinical interventions

Experience in quality improvement, measurement-informed care, and outcomes

Experience working in an acute-care hospital and/or health system setting

**The expected hiring range for this position is $43.57 to $59.91**
