Responsibilities

* Provide direct discharge planning services and assistance with psychosocial services for the patient and family.
* Collaborate with physicians and other members of the healthcare team to develop a discharge plan in relation to the patient's health care needs.
* Utilize broad experience and knowledge of appropriate healthcare resources for patients of all ages.
* Promote continuity through communication of the patient's plan of care.
* Report to the Clinical Resource Management Coordinator.

Qualifications

Job Requirements

* Master of Social Work
* Clinical emphasis from a CSWE accredited graduate school
* Kansas licensure at the LMSW level

Preferred Job Requirements

* Two semesters of graduate field experience in healthcare setting for a new graduate or previous experience in a healthcare setting for an experienced graduate
