Responsibilities:
* Administers medications as stated on physician orders, ensuring strict adherence to Community medication management documentation and procedure, and demonstrating steadfast dedication to resident safety
* Upon thorough assessment, creates personalized service plans tailored to individual care needs while championing resident independence and dignity
* Cultivates communication between physicians, healthcare providers, and family, fostering effective communication pertaining to the physical, mental, and social well-being of our residents
