**Medicaid Eligibility Specialist**

Shift: Monday - Friday

Status: On Call

Location: LIFE New Jersey, 2475 McClellan Avenue Pennsauken NJ 08109

We are looking for compassionate and caring Medicaid Eligibility Specialist to join our LIFE New Jersey team. LIFE New Jersey Healthcare is a member of Trinity Health PACE. Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community.

**What you will do:**

+ Assists participants and or caregivers to initiate the Medicaid approval process.

+ Perform home visits to complete all Medicaid applications on all new enrollees and Re-determinations within a reasonable time frame and submits cases to county Technicians within approved time frames.

+ Work with families and participants in a timely manner to assist them in completing the paperwork needed for Medicaid eligibility.

+ Provide or assists participants with all needed paper work for county technicians and assists the participant or families to provide any additional information as determined and/or required by the county technicians.

+ Complete all re-determinations for participants assigned to them 30 days before the case is due.

+ Responsible for communicating Medicaid rules and regulations to family members and participants (i.e. Straight Medicaid, LTC Medicaid, Spousal Improvishment and Trusts.)

+ Responsible for completing disenrollment forms for HMOs to be disenrolled and communicate progress to the enrollment specialist and department leader.

+ Maintain the confidentiality of all company procedures, results, and information about participants and families

**What you will need:**

+ High school diploma or equivalent required.

+ Must possess a comprehensive knowledge of Medicaid system and benefits.

+ Knowledge of intake practices and geriatric assessment skills.

+ Knowledge of community resources available to seniors and their families.

+ A minimum of one (1) years of experience in working with the frail or elderly population required
