Qualifications

EDUCATION/CREDENTIALS

* At least 3 years Occupational Medicine experience
* Board Certification
* Pennslyvania Medical License
* Unrestricted DEA license for PA
* Must be eligible to participate in Medicare
* Graduate of accredited MD or DO program of accredited university
