Behavioral Health Specialist Job Responsibilities:

* Responsible for providing Behavioral Health (BH) services across Community Care Inc. programs for designated members.


* Serves as a resource to the Interdisciplinary Team


* Provides targeted Behavioral Health Case Management services, as assigned.



Behavioral Health Specialist Job Requirements:

Experience: A minimum of 3 years of experience in providing social work and/or behavioral health case management services to older adults and adults who are physically

and developmentally disabled required; Mental health experience, including provision of individual counseling and support group facilitation with older adults is preferred.

Behavioral Health Specialist Education:

Masters Degree, Social Work, Counseling or related field required.

Certifications/Licensures: Wisconsin Licensed Advanced Practice SW required; LCSW or LPC preferred.

* Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Behavioral Health Specialist Job Schedule:

* Monday to Friday - 8:00AM - 4:30PM
