POSITION SUMMARY:
Responsible for the accurate process of referral/intake services.
EDUCATION/COMPETENCIES:
* Licensed Practical nurse or Registered Nurse
* Possession of a current license to practice as a licensed practical nurse or registered nurse in the State of Connecticut.
* Possession of an individual malpractice insurance policy.
EXPERIENCE:
* 2 years of clinical experience. Home care experience preferred
* Experience with electronic medical records
ESSENTIAL DUTIES and RESPONSIBILITIES:

1. Responsible for identifying and obtaining all necessary information for referrals.
a) Assess W10 and telephone calls from referral sources for appropriateness for home care and hospice care.
b) Accurately enters referral information into Matrixcare.
c) Obtains a verbal order for SOC that includes a description of the patients condition and the services to be provided by the Agency and meets HCFA regulations.
d) Assigns accurate ICD-10 codes to diagnosis.
e) Coordinate all disciplines/services for successful initial assessment.
f) Distributes referral data to appropriate departments and supervisors.
2. Responsible for obtaining accurate insurance information and forwarding to Insurance Specialist.
3. Responsible for obtaining accurate and appropriate documentation for face to face encounters
4. Promotes and maintains positive relationships with all referral sources. Maintains a positive image of agency as first point of client contact.
5. Demonstrates the ability to work effectively with peers.
a) Conferences with appropriate team supervisor regarding assigned referrals
b) Consults with the Referral Center Coordinator regarding referral information..
6. Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per written agreement.
7. Other duties as assigned
