Under the supervision of a Registered Nurse, coordinates the Plan of Care for the patient in the home, hospital or other setting assessing, planning, and implementing. Works cooperatively with other team members in maintaining standards of professional nursing practice and in accordance with age, specific needs and within the scope of practice for License Practical Nurse within New York State.
