Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.

LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.

Primarily responsible for providing transportation/escort services for participants. To assist with the overall transportation coordination needs of participants, including communication as part of the interdisciplinary team. Knowledge and support of the mission and values of Lutheran SeniorLife are important to successful performance, as are commitment to and demonstration of a spirit of teamwork.

Responsible for passengers during transport, providing care in a manner that ensures participant safety, satisfaction and retention.
Operate equipment safely and in compliance with vehicle regulations and Lutheran SeniorLife policies and procedures.
Escort participants to and from residence, (entering residence as required to assist participant), Center, and appointments safely, courteously, and in a caring manner, according to care plan; report any participant or caregiver requests for changes.
Follow established schedules and work with driver to coordinate consistent pick-up times, maintains dialogue with participants during transport.
Safely carry out transfers of participants inside vehicles.
Inspect vehicles and maintain them in a clean and safe condition; notify Transportation Coordinator/Manager of maintenance or repair needs.
Promptly report to Transportation Coordinator/Manager regarding participant-related concerns noted in home or during transport, utilizing communication forms as provided.
Deliver medications, meals, and other supplies to participants ensuring proper processes, procedures and documentation are adhered to.
Work in a safe manner, following safety guidelines and standards.
Attend all required trainings and meetings.
Report accidents and incidents to Transportation Manager immediately, following appropriate procedures and completing documentation as required.
Follow proper infection control procedures and maintain supply of emergency kits.
Participate in day center activities and interacts with participants ensuring staffing ratios are properly maintained.
Secure participants properly in the vehicle to provide safe transportation.
Uses seat/shoulder belts at all times during transport.
Ensures equipment is secured safely and properly during transport.
Provides support to the Driver position and may be required to drive.
Performs other duties and responsibilities as assigned.

High school diploma or general education degree (GED).
At least one (1) year experience in working with the frail or elderly population.
Valid driver’s license.
Driving record free from infractions including, but not limited to, DUI, negligent, reckless, or excessive minor infractions and convictions.
