Job Duties:

Oversight and compliance of program requirements
Interacting with Consumers and Attendants
Coordinating Consumer and Attendant On-boarding
Opening new Consumer cases in their home
Troubleshooting problems and providing resolution
Ability to handle a multi-line phone system
Highly Organized & Detail Oriented
Annual in-person visits to Consumers
Conduct monthly case management calls via phone
Payroll processing related duties

Experience Needed:

Past experience in a medical field preferred
Previous Care Coordination or CDS experience is a plus
Medical office or healthcare experience preferred
Prior management experience preferred.

High School Diploma or equivalent
Valid Drivers License and current insurance
Excellent verbal and oral communication skills
