Job Summary:The duties and responsibilities of the Memory Care Coordinator include overall planning, organizing, developing and directing the overall operation of the Memory Care Department in accordance with current existing federal, state, and local standards, as well as in accordance with the established policies and procedures at the community.  RN or LPN required.

Essential Responsibilities:
  • Maximize fulfilling the caregiving needs of residents who reside in the community.
  • Facilitate the coordination of services and other departments to maintain quality care for residents and offer equal opportunities for team members.
  • Provide direct care to residents in the Memory Care community under the direction of the Community President.
  • Organize, manage, review, monitor, authorize and administer care functions for residents within the Community.
  • Recognize and respond to changes in residents’ conditions and document observations, interventions and outcomes.
  • Oversee and approve the scheduling of personnel as well as the daily assignments of individuals working in the Memory Care Department
  • Monitor the workplace for possible health and safety hazards and assist in the development and conduction of routine health, safety and back care training programs to prevent worker injuries.
  • Review and revise care plans on a regular basis within the guidelines of state and federal regulations and Community.
  • Share ON CALL requirements with other Care Leaders.
  • Share Leader On Duty assignments on a rotational schedule.

Required Knowledge, Skills and Abilities:
  • RN or LPN
  • Three years of experience in long-term care and/or in an AL/MC community.
  • Two years of experience implementing total resident care and care planning.
  • Two years of experience supervising others in a hospital, long-term care facility or other healthcare institution.

Education and Experience:
  • Hold a current, unencumbered RN or LPN license to practice as a nurse in this state.
  • Has advanced education in nursing management administration, geriatric nursing and rehabilitation.
  • Nursing degree from an accredited nursing program, college or university.
  • CPR certification
