Collaborating with an interdisciplinary team of social workers, Support RNs, chaplains, volunteers and more to enhance the lives of our patients and their families.

Compensation: $40.00-$58.67 (DOE). Differentials $5/hour nights & $2/hour weekend.
Sign-on Bonus: $2,000 paid after 90 days of employment.
Location: Redmond, Oregon.
Schedule(s) Available:
Full-Time: Monday-Friday 8:00 am-5:00 pm.
Travel: Local/regional travel by personal vehicle during the working day. Mileage reimbursement is provided.

What You Get To Do:
  • Complete an initial comprehensive and accurate assessment of the patient and family to determine health needs.
  • Develop and update individualized care plans, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions.
  • Coordinate appropriate preventive and rehabilitative nursing procedures, treatments, medication, equipment, and end-of-life comfort procedures as prescribed by the physician.
  • Provide health care instructions and education to the patient and or family as appropriate per the assessment plan and patients caregivers ability to learn.
  • Identify discharge-planning needs as part of the initial care plan development and implement them prior to the patient's discharge.
  • Develop, monitor, and update the Home Care Aid care plan as required.
  • Communicate with all healthcare team members to promote coordinated, efficient care and document coordination of care with all involved with patient care.
  • Participate in weeknight and weekend on-call duties as defined by the on-call policy.
Required Skills & Qualifications:
  • Valid Oregon RN License.
  • Ability to pass a background test, drug screen, and TB test.
  • Graduate of an accredited nursing school.
  • One to two years of recent medical care experience in an institutional setting is preferred. Ideal candidates will possess 1-2 years of home health or hospice experience.
  • Current automobile insurance is required. A valid Oregon drivers license and reliable transportation.
  • Valid CPR Certification.
  • Strong communication and interpersonal skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
