Qualifications:
Required:
- Doctorate degree
- Minimum of 2 years in Primary Care
- Valid NJ Physician License
- Board Certified in Family Medicine or Internal Medicine
- Active DEA and CDS licenses
- American Heart Association Basic Life Support (AHA BLS)
Preferred:
- 1+ years in Geriatric Care, Senior Care, or Long-Term Care (LTC)
- NPI Number
Key Responsibilities:
- Provide direct medical services and consultation to homebound primary care patients
- Monitor patients’ conditions and responsiveness to treatments
- Maintain complete patient records using Electronic Medical Records (EMR) system
- Ensure compliance with NJ Department of Health and other regulatory bodies
- Act as a representative for the agency with patients, families, and other health agencies
- Collaborate on staff education
- Adhere to infection control and safety policies
- Participate in community outreach programs
Schedule and Shift Details:
- 8:30 AM - 4:30 PM, including 1 weekend per month
- Work one summer and one winter holiday
