Excelsior Wellness Center serves a broad and diverse base of individuals and families with the primary aim of identifying goals and making positive steps toward accomplishing them. The Care Coordinator Supervisor works with a multi-disciplinary team including therapeutic recreation specialists, youth and family partners, and education and healthcare professionals to engage and support families. This position provides a collaborative forum to engage a variety of child-serving partners and advocate for the needs of individuals and their families to support the family remaining together and to improve the safety and satisfaction in their lives. This role provides direction in the development and implementation of cross-system care plans that are designed to help the youth and family avoid out-of-home placement and end multi-system involvement. The supervisor will continue to carry a caseload while providing ongoing coaching supervision and support to Care Coordinators. The supervisor will also assist the Director of Care Coordination, in the oversight and day to day management of identified Care Coordinators.

 


How to Qualify:

1. High School Diploma required, BA in counseling or related field preferred.
2. Registered Agency Affiliated Counselor Credential or ability to become registered within 60 days required.
3. Comprehensive knowledge of community resources and public systems.
4. Previous supervisor experience preferred.
5. Minimum three years of working with youth and families.
6. Exceptional communication and interpersonal skills.
7. Must have strong multitasking and organizational skills.
8. Ability to work in a fast-paced environment and make ethical and safe decisions.
9. Ability to manage individual stress and perform essential job functions in a safe and professional manner.
10. Must be able to pass a criminal background check.
11. Must be 21 years or older.
12. Valid state driver’s license.

Core Responsibilities Include:

1. Provides the direction in the development and implementation of community resource programs to achieve desired outcomes for children and families.
2. Understands and actively promotes WISe programs throughout the county, identifying ways in which efforts aimed at improving outcomes for children and families can be coordinated.
3. Recognizes barriers that occur in a system of care and develop and implement practices necessary to sustain system of care improvements.
4. Assesses opportunities and barriers to achieve WISe services across all systems.
5. Develops long and short-term goals and objectives for the development and implementation of agencies WISe program services across all systems of care.

6. Seeks opportunities to effectively communicate with the public and private child and family serving agencies to share information about the program.
7. Serves as liaison between all child and family serving agencies and the WISe treatment team.
8. Coordinates cross-agency communication and planning issues as needed.
9. Directly supervises and coaches Care Coordinators helping develop their skill as they support families in the community; assists and supports effective onboarding of new Care Coordinators.
10. Has the ability to monitor and report Care Coordinator data so it aligns with program and agency standards.
11. Provides administrative assistance as needed to ensure effective program operations; ensures clear communication with direct supervisor to provide suggestions and feedback on program enhancement initiatives.
12. Provides transportation when taking clients/families to activities, shopping centers, school, court, family visits, etc.; drives company vehicles when transporting clients and families.
13. Attends and actively participates in weekly meetings.
14. Complete all required training and demonstrates competency in skills necessary for role.
15. Is aware of and available to all WISe services as needed for additional support in periods of crisis.
16. Conducts necessary trainings as needed and directed by supervisor.
17. Maintains professionalism and unity when communicating with clients, their families, and other representatives; preserves client confidentiality at all times.
18. Attends and actively participates as a representative of Excelsior and its subsidiaries in meetings, both internal and external.
19. Effectively represents Excelsior in the community, to include but not limited to the City of Spokane, the State of Washington and other venues as attended.
20. Adheres to organizational ethical standards, policy and procedures, mission, vision, and values.
21. This position description is not intended to be all-inclusive, and the employee will also perform other reasonably related business duties, across other Excelsior subsidiaries, as assigned.
