Home Health RN Case Manager

Purpose: As a Home Health RN Case Manager at Seasoned Recruitment, you will be responsible for administering skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Manager.
Essential Responsibilities:
- Provide care and services in accordance with the physician-ordered plan of care.
- Conduct initial evaluation visits and regularly reevaluate patients' nursing needs.
- Initiate and update the plan of care as necessary.
- Document patient progression toward clinical goals, plan care conferences, and conduct discharge planning according to agency policy.
- Process orders, notify physicians of patient needs and changes in condition, and complete certification/recertification orders and discharge summaries.
- Coordinate services with interdisciplinary care team and make appropriate referrals to other healthcare professionals.
- Educate and empower patients and their families/significant others for safe home management and discharge.
- Participate in staff meetings, in-service programs, and adhere to established agency policies and procedures.
- Conduct patient care conferences for assigned patients.
- Participate in the onboarding of other nursing personnel, peer reviews, and quality assessment and performance improvement activities.
- Take on-call duty nights, weekends, and holidays as assigned.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with agency-defined time frames.
- Maintain and model REACH values (Respect, Excellence, Accountability, Compassion, Honor).
- Demonstrate regular, reliable, and predictable attendance.
- Perform other duties as required.
Requirements
Education and Experience Requirements:
- Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
- Two (2) years of nursing experience preferred.
- Acceptance of philosophy and goals of this agency.
- Ability to exercise initiative and independent judgment.
Minimum Knowledge, Skills, and Abilities Requirements:
- Basic computer EMR skills.
- NYS Driver's License.
- Proof of car insurance.
License, Registration, or Certification Requirements:
- NYS RN License, BLS, other CPR Training as required.
