JOB SUMMARY: We are seeking a skilled and compassionate Family Physician to join our dedicated healthcare team at our Federally Qualified Health Center (FQHC). As a Family Physician, you will play a vital role in delivering comprehensive primary medical care services to patients of all ages, with a strong focus on underserved and vulnerable populations. Your expertise will contribute to improving the overall health and well-being of our community.

PRIMARY DUTIES AND RESPONSIBILITIES:

  1. Primary Medical Care:
    • Provide comprehensive primary medical care to individuals of all ages, focusing on preventive care, diagnosis, treatment, and management of various medical conditions.
    • Conduct thorough medical assessments, perform physical examinations, and order relevant diagnostic tests to formulate accurate diagnoses.
  2. Chronic Disease Management:
    • Manage and monitor chronic medical conditions such as diabetes, hypertension, asthma, and cardiovascular diseases, collaborating with patients to develop effective treatment plans.
    • Offer patient education on disease management, lifestyle modifications, and medication adherence.
  3. Urgent and Acute Care:
    • Address urgent medical needs and acute illnesses promptly and effectively, including injuries, infections, and other time-sensitive conditions.
  4. Health Promotion and Preventive Services:
    • Advocate and provide guidance on preventive measures, immunizations, screenings, and wellness activities to encourage healthy lifestyles among patients.
  5. Patient-Centered Approach:
    • Foster a patient-centered approach, ensuring effective communication, active listening, and empathy while addressing patient concerns and preferences.
  6. Culturally Competent Care:
    • Demonstrate cultural sensitivity and competence in delivering care to diverse patient populations, recognizing the impact of social determinants of health.
  7. Collaborative Care:
    • Collaborate closely with interdisciplinary teams, including nurses, nurse practitioners, physician assistants, clinical pharmacists, behavioral health specialists, and social workers, to provide comprehensive care.
  8. Electronic Health Records (EHR):
    • Utilize our electronic health record system for accurate documentation of patient encounters, treatment plans, prescriptions, and referrals.
  9. Quality Improvement:
    • Participate in quality improvement initiatives, evidence-based practice, and research efforts to enhance patient outcomes and healthcare services.
  10. Community Engagement:
    • Engage with the local community through outreach programs, health education sessions, and community events to promote health awareness.
  11. Continuing Education:
    • Stay updated on medical advancements, guidelines, and best practices through continuing medical education (CME) activities to maintain clinical excellence.

OFF-SITE WORK:

  1. Off-Site work is required occasionally.
  2. The types of work he/she has the option to perform off-site include (This list may not be exhaustive):
    • Charting
    • Taking call
    • Consulting with other health care facilities or providers

SKILLS/QUALIFICATIONS:

  • Medical degree (MD or DO) from an accredited medical school.
  • Board certification in Family Medicine.
  • Active and unrestricted medical license in the state.
  • DEA registration for prescribing controlled substances.
  • Commitment to serving underserved populations and working in a FQHC setting.
  • Strong clinical and diagnostic skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work collaboratively in a team-based care model.
  • Proficiency in electronic health record systems.
