Responsibilities
  • Conduct confidential, quality intake and Biopsychosocial Assessments.
  • Develop and review Individualized Rehabilitation Plans.
  • Based on the Assessments, make referrals to the most appropriate programs and agencies.
  • Working within a multidisciplinary context, participate in professional meetings, maintain accurate and protected medical info, and provide
    reports as warranted for funding sources.
    Crisis intervention as needed.
Job Requirements

  • Master's Level licensed mental health professional (LGSW, LGPC, LMSW,CPRP, LCSW-C, LCPC)
  • Two years of experience working with a program that provides services to individuals with emotional and behavioral disabilities.
  • Have experience with developing treatment plans and completing assessments.

  • Must hold a valid driver's license with acceptable driving record and provide
    own, insured transportation.
