General Description:

The Behavioral Health Counselor will use a person-centered approach to work with individuals who have complex behavioral health needs. The Behavioral Health Counselor may work collaboratively with the client and outside providers to conduct assessment, crisis intervention, refer and access needed services including support groups, and when needed, provide short term counseling. The Behavioral Health Counselor will provide consultation to ANSCM staff regarding different individual scenarios which do not necessarily elevate to enrollment in the EMHOT program.

Requirements:

ROLE AND RESPONSIBILITIES

  • Conduct initial assessments to determine consumers’ mental health status and needs.
  • Provide short-term one-on-one counseling.
  • Develop and maintain cooperative relationships with community partners such as designated COAs, Behavioral Health Centers/Network, and Emergency Responder’s to effectively utilize all available resources for elders.
  • Utilize various therapeutic techniques such as cognitive-behavioral therapy (CBT), psychodynamic therapy, and solution-focused therapy.
  • Participates in outreach education to promote Elder Mental Health Outreach Team (EMHOT) and educate community partners as needed.
  • Maintain timely and appropriate case documentation according to state regulations, agency standards, other funding sources, and EMHOT requirements.
  • Reviews referrals/intakes and any additional specific documentation in a timely manner to screen and follow up on the referral as appropriate.
  • Maintain caseload of consumers with behavioral health conditions who require short-term counseling or behavioral health interventions deemed beyond the scope of traditional case management.
  • Refers consumers to outside connections such as long-term counseling, psychotherapy, psychiatric nurse practitioner, support groups, or other services available.
  • Participate in team meetings to discuss case management approaches, share professional insights, and develop strategies to improve outreach effectiveness

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • Master’s degree in social work, Mental Health Counseling, or a related field.
  • Licensed as a Licensed Independent Social Worker (LICSW), Licensed Certified Social Worker (LCSW), or Licensed Mental Health Counselor (LMHC)
  • Possesses knowledge of the aging process, community resources and social health intervention techniques desirable.
  • Strong organizational and time management skills.
  • Ability to interpret, understand and adhere to the regulations, standards, and procedures as set forth by the Agency and the Executive Office of Elder Affairs.
  • Ability to communicate professionally, both orally and in writing, to small and large audiences.
  • Ability to conduct in-person assessments with consumers.
  • Ability to effectively cope with a variety of personalities and other stress provoking factors associated with a broad base of people and time constraints.
  • Knowledgeable about and compliant with all applicable state and federal regulations.
  • Ability to work with multiple priorities and deadlines.
  • Ability to handle confidential data according to HIPAA laws and regulations.
  • Ability to maintain computer security.
  • Proficient in Microsoft 365

A valid driver's license and a legally insured and registered motor vehicle are required.

ENVIRONMENTAL:

  • Work is split between indoor office work and traveling outdoors to meetings, trainings and consumers’ homes.
  • Must be able to tolerate odors and pollutants including but not limited to smoke, air fresheners, pet dander, personal odors, etc. from a variety of in-home conditions.
  • Must be able to tolerate by-products of office machine operation.
  • Must be able to tolerate heat and cold of seasonal changes and indoor temperatures.
