Summary
As the lead agency for the SPA 6 Coordinated Entry System (CES) for Single Adults, HOPICS is responsible for identifying the most vulnerable homeless individuals and equitably matching these individuals to housing subsidies through the prioritization process established by the Continuum of Care. In partnership with CES funded partners within SPA 6, the CES Time Limited Subsidy Matcher will effectively match and track housing program placements as well as identify opportunities for data and system improvement of the CES. In addition, the CES Time Limited Subsidy Matcher will model the effective use of CES by supporting, increasing, and improving local participation in Homelessness Management Information System (HMIS), while working closely with our interim housing providers and housing navigators to streamline client linkages to available housing opportunities for streamlined lease ups. Under the direct supervision of the CES Matching Supervisor, the CES Time Limited Subsidy Matcher is responsible for the following essential functions.
Essential Functions
    Understand and accurately articulate the HUD definition of homelessness and chronically homeless.
    Understand and be able to articulate services provided through the SPA 6 CES for Single Adults.
    Understand and be able to articulate the various housing resources, subsidies, and eligibility requirements.
    Understand and accurately articulate the LAHSA CES Matching Policy.
    Understand and accurately articulate the Interim Housing and Housing Navigation Program.  
    Coordinate time limited subsidy matches and data sharing across CES subcontractors and participating agencies. 
    Cultivate continued collaboration among interim housing, housing navigation and time limited subsidy providers to enhance referral opportunities and rapid placement.
    Provide ongoing support and conduct trainings/presentations regularly to acclimate new staff to the Time Limited Subsidy Matching process.
    Attend all Time Limited Subsidy specific meetings and trainings.
    Attend weekly case conferencing meetings in SPA 6 for increased and accurate matching to available time limited subsidy resources.
    Coordinate services, trainings and distribution of CES matching information for SPA 6.
    Act as an advocate for system coordination including collaborating with regional leads and partner agencies to identify specific process barriers and create potential solutions.
    Facilitate case consultation meetings with community providers and monitor the collaborative efforts between those partners who are matched up to maximize their navigation slot usage. 
    Provide HMIS technical assistance on the linkages and the referral process for the system flow of Interim Housing to Housing Navigation to Time Limited Subsidies.  
    Maintain accurate documentation of matches in HMIS, Interim Housing and Housing Navigation trackers. 
    Maintain an updated log of individuals being matched to available Time Limited Subsidy slots.
    Track and report weekly the number of homeless Time Limited Subsidy slots.
    Submit weekly reports to the CES Matching Supervisor of the number of Time Limited Subsidy slots.
    Set and manage the timeline for project deliverables and report to the community at large and our funders.
    Manage and maintain the Community Queue system registry in HMIS to track the allocation of Time Limited Subsidy slots within SPA 6.
    Maintain detailed records and communication logs to build narratives out of data that showcase the importance of collaboration through outcomes and achievements.
    Establish and maintain effective working relationships with the general public, elected and appointed officials, members of the collaborative and key housing provider players. 
    Participate in CES infrastructure building within the organization and in the community.
    Maintain ongoing knowledge of the eligibility criteria and documentation standards for all short, medium, and long term permanent housing subsidy programs.
    Contribute to the obtainment of all CES contractual performance targets and the obtainment of division wide goals and performance outcomes.
    Some evenings and weekends required.
    Perform any other appropriate responsibilities as assigned by Management.
    Maintain accurate files/records on activities, client services in compliance with HIPAA, CFR 42 and other funding requirements for auditing purposes.
    Attend meetings and/or regional events associated with other projects as assigned by the supervisor for purposes of obtaining additional housing resources, securing units, etc.
    Maintain appropriate boundaries with staff, clients and community partners. 
    Adhere to SSG’s Code of Ethics and HOPICS’ Core Values.  
    Represent the Agency in a professional manner at meetings and community events.

Minimum Qualifications - Knowledge, Skills and Abilities Required 
    High School diploma and 3 years’ experience working in homeless services or related field OR bachelor’s degree and 1-year experience in homeless services or related field.
    Working knowledge of short and long term permanent housing subsidies in LA County.
    Working Knowledge of Homeless Service Providers and Coordinated Entry System.
    Able to communicate and interact with subcontractors, housing providers and community partners in a culturally competent manner.
    Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills
    Self-starter with the ability to stay ahead of the curve and thrive in fast-paced work environment.
    Able to work effectively with a wide range of constituencies.
    Strong interpersonal and team building skills.
    Able to work with and lead community groups and case managers.
    If in recovery, a minimum of three (3) years of being drug and alcohol free is MANDATORY.
    Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.
    Verification of Employment Eligibility and Background Clearance.
    Ability to communicate effectively, both written and orally.
    Valid driver’s license and reliable transportation.
    Current proof of Auto Insurance required.
    TB test required (Not more than (3) months prior to or (7) days after date of hire and renewal annually thereafter.
    CPR and First Aid Certification required within 30 days of employment with company.
    Basic computer word processing skills required.
    Ability to understand and carry out oral and written direction.

Non-Essential Qualifications (optional) – Knowledge, Skills and Abilities 
    Strong background or experience with Microsoft Office and Excel, HMIS, and other database programs is desirable.
    Knowledge of various community based treatment providers is desirable.
    Ability to quickly grasp concepts and apply them to solve complex social problems is desirable.
    Ability to articulate and strategically pursue new ideas to improve homeless systems is desirable.
    Ability to think critically and pay attention to detail is desirable.
    Strong Public Speaking Skills

Supervisory Responsibilities:
This position does not have any supervisory responsibilities.
Environmental Conditions (Working Conditions)
    The environment for this position is office and field based.
    Must be able to work in an environment with many priorities, busy, and fast paced.
    Must be able to problem solve, adapt to changes that are unpredictable.
    Must be able to work with highly aggressive clients.
    Must be able to work in various shelters environments.
    Exposed to highly aggressive clients: must communicate too many sources including courts and outside community agencies: sometimes noisy, loud and disruptive clients.

Physical Requirements
    In the course of performing this job, the incumbent typically spends time sitting, standing, walking, speaking, thinking, listening, and carrying (20lbs). Must sit at computer for data entry and reports: sometimes spend time driving and walking and carrying equipment/files/charts. Assisting client with moving their personal belongings. Speak to staff and outside agencies in public meetings and Provider Meetings. Represent the Agency in a professional and positive manner
