ABOUT THE ROLE

The Nurse Practitioner/Physician Assistant is responsible for providing relief from the symptoms of patient’s chronic progressive illnesses and preventing hospitalization through triage and coordination of care. The Nurse Practitioner/Physician Assistant is a mobile provider that is responsible for providing relief from the symptoms of patient’s chronic progressive illnesses and preventing hospitalization through triage and coordination of care.

Core Job Functions:

  • Provide advanced primary and palliative care to patients at their residence
  • Develop treatment plans to provide symptom management.
  • Provide on-call support and work closely with triage team to prevent hospitalizations and provide patient support.
  • Collaborate with health care partners and referral sources to promote and sustain community relationships.
  • Coordinate with patient’s care team and referral sources to improve patient care and prevent readmissions.
  • Arrange for comfort measures such as pain management, home health and spiritual care.
  • Assist in end-of-life discussions and hospice transitions, if necessary.

* This schedule will be Monday- Friday 8am-5pm*

*This position will be located in: Phoenix, Ahwatukee, Maricopa, and Casa Grande*


ROLE REQUIREMENTS

  • Master’s degree in nursing from an accredited program with current license to practice professional nursing or graduate from an ARC-PA accredited physician assistant program.
  • Board certified advanced nurse practitioner or Physician Assistant
  • Active, state license in good standing
  • Current DEA
  • CPR certification
  • Minimum of 2 years of health care experience in hospital, medical outpatient clinic, home health, hospice or specialty care setting.
  • Must be a licensed driver with an automobile that is in good working order and demonstrate ongoing insurance in accordance with state and or organization requirements.
