CARE COORDINATOR
REPORTS TO: Director of Care Coordination
POSITION REQUIREMENTS: The Care Coordinator must possess a minimum of a bachelor’s degree in human services, social work, or is a registered nurse. A bachelor’s degree in social work is preferred. It is preferable that the care coordinator have two years of satisfactory experience in the human services field working with the elderly. Must have computer knowledge and experience.
SUMMARY: Will assess client needs and coordinate services among all providers. The care coordinator will also develop resources to meet the older adult’s needs. Prepares reports and other materials as required. Maintain caseload and documentation on client files; performs data entry. Routine travel in service area is required. Significant public contact is required and position requires some independent decision-making and action.
