Essential Duties and Responsibilities:

  • Maintain two cases billing on average 10hrs per week.
  • Collaborate with clinical manager in regard to assessments being conducted, submission to kepro, and case management needs.
  • Provide case management services to families that are awaiting approval from kepro
  • Document, utilizing progress notes case management services provided to client’s awaiting approval from kepro.
  • In providing case management services the staff should bill no less than 3hrs per week and no more than 12hrs per week as deemed necessary by the clinical manager.
  • If authorization is approved aid in appropriate transition of client to permanent counselor
  • If authorization is denied conduct appropriate discharge and closure with the family
  • Attend all staff meetings and trainings.
  • Assist program manager with chart audits and quality assurance on programmatic level.
  • Adhere to all policies and procedures and complete other tasks deemed necessary by the program manager/clinical manager for the successful operation of the program.
  • Assessing client's mental health status through interviews or observations and developing Individual Service Plans and assisting in making referrals to appropriate services.
  • Provide crisis intervention services to clients who are a danger to themselves or others.
  • Accurate, descriptive, and timely documentation must be maintained and submitted for all client interactions..

Qualifications: Completion of a bachelor’s degree in social work, sociology, psychology, or other human service field and one year of experience working in Intensive In-home and/or bachelor’s degree and five years’ experience working with similar programs for serious emotionally disturbed children. A valid Virginia Driver’s License is required for employment.


Summation: The case management needs that will be provided will vary from case to case. In this position it will be imperative that the case manager communicate consistently with the clinical manager to acquire the most hours possible based on need and Medicaid standards (12hrs). The position is considered a supervisory position and should only be afforded to candidates that have demonstrated an exemplary ability to provide Intensive In-home services.

Reasoning Ability: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.


Computer Skills: To perform this job successfully, an individual should have knowledge of Spreadsheet software and Word Processing software.
