How will this role have an impact?

The Clinical Services Coordinator acts as a clinical liaison between members, providers, and health plans. This position will be responsible for identifying urgent/emergent health concerns from the Signify Health providers to the member's health plan, primary care physicians, and/or internal Signify Health team members, while effectively documenting case management needs to the health plan. The Clinical Services Coordinator must have a strong understanding of medical terminology and excellent critical thinking skills.

This role will report to our Clinical Services Manager.

What will you do?

  • Follow client customary referral request guidelines to generate health plan referrals
  • Communicate with Signify Health providers directly regarding urgent/emergent referrals
  • Communicate directly with select member's health plans to report care management issues
  • Maintain daily performance quota as defined by the department
  • Provided support for in-field Signify Health Clinician inquiries via Inbound calls
  • Work effectively with frequent interruptions

We are looking for someone with:

  • High School Diploma or equivalent
  • MA or LVN required
  • 5+ years' experience working in the back office of a medical office or other clinical setting
  • Accurate data entry skills
  • Typing speed min 45 WPM
  • Strong attention to detail

The base salary hiring range for this position is $16.44 to $28.08. Compensation offered will be determined by factors such as location, level, job-related knowledge, skills, and experience. Certain roles may be eligible for incentive compensation, equity, and benefits.
