Job description:

We are seeking a dedicated and compassionate Home Health RN to join our growing team in Galena, IL. In this rewarding role, you will play a pivotal role in providing exceptional care to patients recovering from illness, injury, or surgery. You will work collaboratively with physicians, therapists, and other healthcare professionals to create and implement individualized care plans that promote healing, independence, and well-being.

Responsibilities:

  • Conduct comprehensive patient assessments to identify needs, develop individualized plans of care, and prioritize interventions based on physician orders.
  • Provide a wide range of skilled nursing care, including:
    • Wound care and ostomy management
    • Medication administration (including complex medications)
    • Pain management
    • Catheterization
    • Venipuncture
    • Intravenous (IV) therapy (if qualified)
    • Other interventions as ordered by the physician
  • Educate patients and families on self-care, disease management, and medication administration techniques.
  • Monitor vital signs, patient condition, and response to treatment, reporting any changes or concerns to the healthcare team.
  • Document care meticulously and accurately in accordance with established standards and regulations.
  • Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure continuity of care and achieve optimal patient outcomes.
  • Promote a safe and healthy home environment for patients by identifying and addressing potential hazards.
  • Participate in ongoing education to stay current with best practices in home health care.

Qualifications:

  • Active and unencumbered Registered Nurse (RN) license in the state of Illinois.
  • Minimum 1 year of experience in home health care or a related acute care setting (preferred).
  • Strong clinical assessment, critical thinking, and decision-making skills.
  • Excellent communication, collaboration, interpersonal, and organizational skills.
  • Compassionate and empathetic demeanor when working with patients and families in their homes.
  • Ability to work independently, manage your time effectively, and prioritize tasks in a fast-paced environment.
  • Proficient in documentation and medical record keeping using electronic health records (EHR) preferred.
  • Valid driver's license and reliable transportation required.
  • Current Basic Life Support (BLS) certification. ACLS certification preferred.
