OhioRISE Care Coordinator

  • Degree and Experience:
    • High School Diploma / GED + 3 years of relevant experience
    • Bachelor’s Degree in Social Work, Psychology, Criminal Justice, or Human Services + 2 years of experience
    • Master’s Degree in Social Work, Counseling, Psychology, Criminal Justice, or Human Services + 1 year of experience
  • Experience: Must be in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a human services field, providing community-based services to youth and their caregivers.
Primary Job Responsibilities:
  • OhioRISE Initiative: Part of Ohio Medicaid’s program to provide high-level care coordination and tailored services for youth and families with complex care needs.
  • Care Coordination:
    • Serve as the point of contact for assigned OhioRISE youth.
    • Comprehensive care coordination for youth and family service needs.
    • Collaborate across systems including education, behavioral health, juvenile justice, developmental disabilities, hospitals, and family councils.
  • Service Planning:
    • Provide structured service planning and care coordination through high fidelity wraparound.
    • Conduct care coordination activities within OhioRISE guidelines.
    • Complete comprehensive CANS assessments and ongoing assessments.
    • Convene and facilitate child and family teams for care plan development and review.
  • Crisis and Transition Management:
    • Develop crisis safety plans.
    • Perform referrals and linkages to appropriate services and supports.
    • Facilitate discharge and transition activities for youth in various settings.
  • Advocacy and Empowerment:
    • Advocate for client and family interests.
    • Empower clients and families to identify needs and resources.
  • Documentation and Compliance:
    • Complete and maintain required paperwork.
    • Maintain professional licensure (if applicable).
    • Demonstrate proficiency in Electronic Health Records.
    • Complete required OhioRISE/CARF/Child Focus training.
    • Participate in staff development and team meetings.
  • Professional Conduct:
    • Present professionally and ethically.
    • Work respectfully with culturally diverse populations.
    • Display professionalism, organizational, and time management skills.
    • Maintain a 1:10 Care Coordinator to ICC youth roster or 1:25 Care Coordinator to MCC youth roster.
