Job Description:

We are actively seeking a Bilingual Home Care Coordinator to join our team in Brooklyn, New York. As a crucial member of our in-house staff, your primary responsibility will be to efficiently staff cases, schedule aides, and ensure the seamless delivery of care to patients in their homes.

Responsibilities:

  • Staffing Coordination: Efficiently allocate and schedule home care aides for patient assignments, ensuring optimal staffing levels.
  • Patient-Aide Scheduling: Coordinate scheduling to guarantee aides are assigned and arrive punctually at patients' homes.
  • Patient Advocacy: Act as the liaison between patients, families, and home care aides to address concerns, provide information, and ensure a smooth care experience.
  • Bilingual Communication: Utilize proficiency in both Spanish and English to facilitate clear communication between all parties involved.
  • Collaboration: Work closely with internal teams, healthcare professionals, and external partners to maintain effective communication and coordination.

Requirements:

  • Bilingual Skills: Proficient in both Spanish and English, with clear and effective communication skills.
  • Previous Experience: Demonstrated experience in staffing and coordinating home care cases, preferably in a bilingual capacity.
  • Communication Skills: Excellent interpersonal skills for effective interaction with patients, families, and healthcare professionals.
