FLSA Status: Exempt

Position Overview

The Behavioral Health Coordinator plays a vital role in managing and coordinating behavioral health services within the organization.

This includes overseeing patient care plans, facilitating communication between patients and healthcare providers, providing direct patient care, and assisting with developing policies/procedures/pathways to ensure the smooth operation of behavioral health programs.

The candidate will be skilled in and responsible for both clinical and administrative tasks.

Qualifications

Education, Experience, Skills

  • Minimum of Master’s Degree in Nursing (MSN)
  • Current, unrestricted license to practice as a Nurse Practitioner (NP) in the State of Michigan
  • Current, unrestricted NP National Board Certification from one of the following organizations: American Academy of Nurse Practitioners, American Nurses Credentialing Center
  • Current, unrestricted DEA controlled substance registration
  • Minimum 5+ years of experience working as a Nurse Practitioner
  • Minimum 3+ years of practice experience as a Nurse Practitioner in a behavioral health inpatient or outpatient healthcare setting.
  • BLS/BCLS certification (may be obtained within 3 months of employment)
  • Experience in case management, care coordination, and/or patient advocacy (highly desirable).
  • Knowledge of behavioral health services, treatment modalities, and community resources.
  • Strong organizational and multitasking abilities.
  • Excellent communication and interpersonal skills.
  • Proficient in healthcare management software and electronic health records (EHR) systems.
  • Strong problem-solving and decision-making skills.
  • Ability to work both independently and as part of a multidisciplinary team.
  • Essential Job Functions, Key Roles, and Responsibilities

Patient Care Coordination:

  • Conduct/Coordinate in-person or virtual patient assessments/consultations, and provide treatment and follow-up care.
  • Diagnose and treat acute, episodic, and/or chronic conditions.
  • Prescribe and order necessary medications and treatments; this will include appropriate use of approved controlled substances.
  • Develop, implement, and monitor individualized care plans.
  • Serve as a liaison between patients, families, and healthcare providers to ensure cohesive care.
  • Ensure timely and effective service delivery, addressing any barriers to care.

Administrative Support:

  • Assist in the development and implementation of behavioral health programs and initiatives.
  • Maintain accurate patient records and ensure proper documentation practices.
  • Schedule patient appointments, coordinate referrals, and manage patient follow-ups.
  • Handle patient inquiries and resolve any concerns or issues promptly.

Team Collaboration:

  • Work closely with community behavioral health organizations, therapists, counselors, social workers, and other healthcare providers/clinicians to support patient care.
  • Participate in team meetings and case reviews to discuss patient progress and treatment plans.
  • Provide support and coordination for clinical workshops, and patient/staff educational programs.

Community Outreach:

  • Develop and maintain relationships with community organizations, schools, and other stakeholders as applicable.
  • Coordinate outreach efforts to promote behavioral health awareness and services.
  • Represent the organization at community events, health fairs, and professional meetings.

Compliance and Quality Assurance:

  • Ensure compliance with all relevant healthcare laws, regulations, and accreditation standards.
  • Implement quality assurance measures to maintain high standards of care.
  • Conduct regular audits of patient records and care plans to ensure compliance with organizational policies.

Data Management and Reporting:

  • Collect and analyze data on patient outcomes and program effectiveness.
  • Prepare reports on organizational activities, patient progress, and program performance.
  • Assist in identifying opportunities for program improvement and ongoing development.

Working Conditions

  • Hours of Work: Typical hours are during the day, Monday-Friday, with rotating evening and weekend coverage (virtually only).
  • Overtime Expectations: No overtime is expected. Regularly working above and beyond expected hours should be discussed with a coordinator, manager, or HR to address potential delegation or changes to compensation.
  • Work Environment: This role typically involves working in an office environment, with potential exposure to clinical settings. Regular use of medical equipment and technology is required.
  • Physical Factors: Includes sitting, standing, walking, squatting, reaching, lifting, and bending.
  • Travel Requirements: Travel may be required, with frequency and associated expenses or benefits to be discussed.
  • On-Call Responsibilities: Rotating on-call duties for virtual and phone coverage in the evenings and weekends, required between the hours of 7am-9pm, excluding major holidays.
