Visits patients in counties - Jackson, Swain, Cherokee, Macon, Graham, and Clay - Mileage Reimbursement included
Pay Rate: $18.00 - $22.00 per hour - Varies based on experience and credentials
Position Summary:
Provide excellent, professional day-to-day hands-on personal care to individuals with chronic, serious, or life-limiting conditions. CNA must exhibit competencies required by law and abide by Four Seasons policies in providing personal care to patients, as directed by the written plan of care.
Essential Duties & Responsibilities:·Works to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies
- Practice within the scope of license, certification or registration, where applicable, and as required by law and regulations as delegated to and directed by the registered nurse:
- Participates in the coordination of the patient’s hospice care in accordance with professional standards of practice
- Collects data for ongoing interdisciplinary comprehensive assessments
- Participates in the development and evaluation of the plan of care by reporting observations
- Contributes to patient and family counseling and education
- Perform duties in compliance with Medicare Condition of Participation
- Practice safety and infection control methods
- Provision of hands-on personal care
