MAKE A DIFFERENCE AS A INTENSIVE CARE COORDINATION CASE MANAGER - CHILD AND ADOLESCENT

As an Intensive Care Coordination Case Manager at Hampton-Newport News Community Services Board, you will play a vital role in providing comprehensive assessment, treatment planning, and coordination for children with Serious Emotional Disturbance (SED) or Intellectual or Developmental Disability (IDD). Your responsibilities will include ensuring continuity of services, monitoring progress towards treatment goals, and promoting family and community involvement in crafting innovative service plans. You will work towards serving children in the least restrictive and least intensive level of care possible while focusing on their individual needs. Your role will involve a team-oriented approach, outreach, advocacy, crisis intervention, and daily living support to ensure the well-being and success of the children and families you serve.

To excel as an Intensive Care Coordination Case Manager at Hampton-Newport News Community Services Board, candidates must possess a Bachelor's degree in Human Services (such as Social Work, Psychology, or Counseling) along with a minimum of two years of experience in providing children's mental health care management services. Proficiency in utilizing the DSM V for diagnosing purposes is highly preferred, showcasing a strong understanding of mental health disorders and treatment planning. A valid driver's license is essential for this role, as it may require traveling to different locations to meet with clients or coordinate services effectively.

Excellent communication skills, a compassionate approach, and the ability to work collaboratively in a team environment are key attributes needed to succeed in this position.

Knowledge and skills required for the position are:

  • Bachelor's degree in Human Services (such as Social Work, Psychology or Counseling)
  • Two (2) years of experience providing children's mental health care management services.
  • Knowledge and experience with DSM V diagnosing preferred.
  • Valid Drivers' Licenses.
