Hospice of the Piedmont is on the lookout for a full-time Certified Nursing Assistant (CNA) to join our Rapidan Team. We need someone compassionate and dedicated to serve the counties in the northern region of our territory, in and around Culpeper, VA. You will be providing exceptional hospice care to patients in their homes during their final stages of life. As a CNA in our hospice care program, you will play a crucial role in providing comfort, support, and assistance to patients and their families during this challenging time. This is an opportunity to make a meaningful impact and bring comfort to those in need.


Responsibilities:

  • Provide direct patient care in accordance with the care plan developed by the interdisciplinary team, including assistance with activities of daily living (ADLs), personal hygiene, and comfort measures.
  • Observe and report any changes in patient condition or behavior to the hospice nurse.
  • Assist with mobility and transfer of patients as needed.
  • Document all care provided and any changes in patient condition accurately and timely.
  • Provide emotional support and companionship to patients and their families.
  • Collaborate with the interdisciplinary team to ensure the highest quality of care.
  • Maintain a clean and safe environment for patients.
  • Respect and uphold patient rights and confidentiality.


Requirements:
  • Valid certification as a Certified Nursing Assistant (CNA) in Virginia
  • Prior experience in hospice care or a similar setting is preferred but not required.
  • Demonstrated knowledge of principles and practices of direct patient care.
  • Compassionate and empathetic nature with the ability to provide emotional support.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Strong organizational skills and attention to detail.
  • Ability to maintain confidentiality and professionalism.
  • Physical stamina to perform the duties and responsibilities of the position.
