Case Manager/Case Coordinator

The Case Manager works with rehabilitation patients with various disabilities including, but not limited to: spinal cord injury, brain injury, cerebrovascular accident, neurological disorders, orthopedic conditions, etc. Monitors and manages clinical and financial coordination of treatment plan of assigned patients to ensure timely, cost-effective, individualized service delivery.
Responsibilities
- Communicate effectively with doctors, patients, health care entities to gather information, provide updates, and coordinate patient care
- Ensure all necessary documentation and paperwork is completed in a timely and accurate manner
- Assist in coordinating patient care and follow-up appointments.
- Provide support and guidance to patients and their families throughout the case management process
- Understands, complies with and makes individual centered decisions based on all required areas of specific programs that apply to case management services
Requirements
- Valid Driver's License with clean driving record
- Two year's direct service experience with persons with disabilities
- Experience in completing assessments, service plan development and monitoring
- Effective organizational and communication skills

Long term care/Rehabilitation center
