Duties & Responsibilities:
Provides advanced advocacy, assessment, and case management services.
Conducts outreach, intake, and individual/group supervision of program participants.
Develops comprehensive case management plans to address various challenges.
Counsels individuals or groups, establishing service/treatment plans and monitoring progress.
Collaborates with community resources, offering financial assistance and management support.
Develops and maintain professional relationships with social service organizations and government agencies.
Qualifications:
Bachelor's Degree from an accredited institute of higher learning.
Minimum of two years of experience in casework, human services, or related field.
Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Human Care Services.
Knowledge/Skills/Abilities (K/S/A):
Ability to read and interpret documents such operating and maintenance instructions, and procedure manuals.
Ability to write routine reports and correspondence.
Ability to speak effectively before groups of customers or employees of organization.
Ability and comfortability working with people of varying socioeconomic backgrounds.
Excellent communication skills. Strong networker, convener, relationship builder and communicator with experience collaborating with diverse work teams and stakeholders.
