PRIMARY PURPOSE: The Social Services Coordinator will work with the Residents of Mountain View Community by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs.

The Social Services Coordinator is responsible for following policies and routines that enable Residents to maximize their individuality, independence and dignity. This shall provide Residents with the highest practical level of physical, mental, spiritual and psychosocial well-being and quality of life. This position assists in Resident Admissions and Discharges in line with regulation and Mountain View policy. In addition, the Social Services Coordinator is responsible for the timely, accurate and complete submission of all that is necessary to ensure compliance with all applicable rules and regulations including those required for third party reimbursement to Mountain View.

DESCRIPTION OF DUTIES AND RESPONSIBILITES:
PRIMARY FUNCTIONS:

  • Conducts MDS interviews, assessments, care plans, review charts, writes notes and attend and actively participates in meetings as directed.
  • Meets with Residents, Families and Resident representatives as needed at various times as needed including early mornings, evenings and weekends.
  • Shall coordinate Resident Admissions and documentation as required by regulation, Law, or facility need.
  • Coordinate the Resident discharges planning process and make referrals for appropriate home care services prior to the Resident’s return to the community.
  • Shall maintain the Admissions wait list including contact with families of those on the list, ensuring it is accurate and up to date, and proving ongoing support to those on the list.
  • Shall be available as directed to travel for in-home meetings/assessments along with assessments in other long term care and acute care facilities.
  • Review Facility policies and procedures as part of MVC’s interdisciplinary team to assure compliance with state and federal regulations.
  • Participate in reviewing and setting policies concerning Resident care and quality of life.
  • Participate in developing MVC’s social work policies.
  • Develop, maintain and utilize a listing of current community resources that are useful to
  • Residents and their families/significant others.
  • Participate in quality assurance and interdisciplinary team meetings.
  • Understand and meet all government requirements for social service documentation.
  • Document progress in meeting the psychosocial needs of Residents.
  • Complete a social history and psychosocial assessment for each Resident that identifies social, emotional, spiritual and psychological needs.
  • Participate in the development of a written, interdisciplinary plan of care for each
  • Resident that identifies the psychosocial needs/issues of the Resident, the goals to be accomplished for those needs/issues, and the appropriate social worker interventions.
  • Ensure or provide therapeutic interventions to assist Residents in coping with their transition and adjustment to a long-term care facility, including their social, emotional, spiritual and psychological needs.
  • Ensure or provide support and education to Residents/family members/significant others to assist in their understanding MVC issues in addition to referring them to the appropriate social service agencies when MVC does not provide the needed services.
  • Provide clinical interventions to address catastrophic events that occur during the
  • Resident’s stay in the facility.
  • Works with the interdisciplinary team and administration to promote and protect
  • Resident rights and the psychological well-being of each Resident. Prevent and address
  • Resident abuse as mandated by law and regulation.
  • Identify community changes and opportunities such as legislation, regulations and programs that affect MVC Residents
  • Required attendance during emergency situations to assist in support roles as directed by Supervisor or Administrator. Emergency situations include weather events and any situation that could result in reduced staffing.
  • Required attendance at general Resident and/or Staff events as directed by the
  • Administrator including emergency situations to assists in support roles.Coordinate Family education, communication and support groups
  • Works collaboratively with a multidisciplinary team in advancing special programs and
  • services for Residents.


QUALIFICATIONS/EDUCATION:

  • Requires a minimum of a Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, Rehabilitation Counseling or other related disciplines.
  • A minimum of three years of supervised social work experience in a long-term care setting working directly with elders.
  • Requires demonstrated competence in effective communication (both written and verbal), assessment and social work methods and techniques. Requires a demonstrated ability to work effectively with a variety of disciplines in both an individual and team setting. Requires a knowledge of applicable state and federal nursing home regulations, compliance requirements and professional standards of best practices. Must possess and maintain a valid Driver’s License.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES

  • Individuals must possess these knowledge, skills and abilities or be able to explain and demonstrate that the individual can perform the primary functions of the job, with or without reasonable accommodations, using some other combination of skills and abilities and to possess the necessary physical requirements to safely perform the primary functions of the job.
  • Must be able to deal well with stressful and difficult situations.
  • Must have an excellent ability to multi-task in an ever changing nursing home environment.
  • Subject to frequent interruptions and the ability to respond in a calm and supportive manner to potentially disruptive situations.
  • Must possess the ability to work tactfully and patiently with residents, family, visitors and staff.
  • May be subject to hostile or emotionally upset residents, family members or visitors.
  • Ability to establish and maintain an effective working relationship with other Staff Members,
  • Department Heads, and the General Public.
  • Must possess strong organizational and communication skills and the ability to interact with staff, residents, and families in a professional manner.
  • Must be able to sit, stand, bend, lift and move intermittently throughout the day in order to speak with residents in wheelchairs and varied locations/situations.
  • Ability to lift up to 20 pounds and in an emergency evacuation of residents, must be able to safely push an occupied wheelchair for a minimum of 500 feet.
  • Must possess proficient computer skills including Word, Excel and Outlook. Experience with clinical software and/or MDS software desired.
  • Must possess reading and writing skills and the ability to express, receive or exchange ideas verbally and in writing.
  • Ability to demonstrate a clear understanding of the Mountain View Community principles, policies and procedures and follow all established safety procedures, policies, and general safety practices when performing duties.
  • Maintain neat and professional appearance.
  • May be subject to exposure to infectious diseases and / or waste.
  • May be subject to pre and post-employment drug testing.
  • Subject to criminal background check, state BEAS check and all state and federal hiring requirements.

WORK SCHEDULE
Must work flexible hours in order to be available to Residents, Resident Families and Staff on all shifts with the typical hours of the position being Monday through Friday from 8:00 am to 4:00 pm.
