What You'll Do:

  • Provide service coordination that includes care coordination, client advocacy, and a framework of action for the client to achieve specified goals.
  • Meets regularly with clients to implement housing plan, stabilize clients’ own home and then develop life plan goals.
  • Maintains relationships with landlords and community resources to connect clients with
    housing opportunity.
  • Document service coordination activities throughout the continuum of care.
  • Assess Service Plan and in consultation with the client, make appropriate changes to the
    Service Plan to ensure progress toward treatment and/or recovery goals.
  • Arrange for advocacy services around issues of housing, client or family involvement with court system or other services relevant to the client’s well-being.
  • Maintain updated list of local agency contacts and list of key documents required for each housing and service offering to help expedite process.
  • Maintain ongoing contact with client and involved support systems to ensure adherence to the treatment plan.
  • Provide transportation support to minimize barriers to continuum of care.
  • Establish and maintain relations with civic groups, agencies, and other professionals, governmental entities, and the community-at-large to ensure appropriate referrals, identify service gaps, expand community resources, and help to address unmet needs.
  • Apply confidentiality regulations appropriately.
  • Provide reports and paperwork as required in a timely manner.
  • Attend all meetings as required.
  • Attend all core trainings for drug and alcohol case management and addiction trainings that pertain to the role of housing case manager.
  • Attend community outreach events as needed.
  • Maintain identified certifications and minimal training hours annually
  • Performs miscellaneous job-related duties as assigned.

Requirements:

  • Healthcare Knowledge: A fundamental understanding of healthcare services and basic medical terminology.
  • Tech-Savvy: Proficiency in basic data entry computer skills.
  • Strong Interpersonal Skills: Ability to communicate courteously and effectively with individuals from diverse backgrounds.
  • Team Player: An industrious and cooperative nature to foster harmonious working relationships with clinic management, physicians, clinicians, and the public.
  • High School Diploma: High school diploma is necessary or an equivalent
  • Driving: Valid State Drivers License

Preferred Qualifications:

  • Associates Degree
  • Prior CPR/First Aid certification
  • One (1) year of working experience in medical or physician office setting.
  • Knowledge of The Joint Commission regulations
  • Bi-lingual in Spanish, Nepali, French, Arabic
