Who We're Seeking: Are you a dedicated and compassionate Medical Social Worker (MSW) looking for a fulfilling opportunity to make a real difference in the lives of hospice patients? Your Responsibilities: As a member of our dedicated team, you'll:

  • Directly engage with patients in the comfort of their own home, providing one-on-one support.

  • Participate in team meetings and collaborate with supervisors to identify challenges, and foster supportive and innovative solutions.
  • Deliver evidence-based interventions tailored to address psychosocial challenges impacting clients’ medical care.

Requirements:

  • Master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education
