Position Overview:

High-Fidelity Wraparound is an evidence-based, intensive care coordination model to support children and youth with serious behavioral health challenges in living successfully in the community with their families. All members served by the Hi-Fidelity Wraparound Care Manager are enrolled in the NYS Department of Health Children’s Health Home Care Management (Community Outreach & Care Coordination) program. The High-Fidelity Wraparound Care Manager’s primary responsibility is to assist in developing a cohesive Child and Family Team composed of the different people who provide services and support to the member and their family. Child and Family Team members may include healthcare, social care needs providers, school personnel, and the family’s natural supports. The Child and Family Team is responsible for creating and carrying out an integrated and comprehensive Wraparound Plan grounded in the youth and family’s individualized needs and strengths. It is strongly informed by their cultural values, choices, and vision for the future. Hi-Fidelity Wraparound Care Manager is responsible for ensuring the model is implemented with fidelity for each member and family while also carrying out all elements of the Children’s Health Home Care Management (Community Outreach & Care Coordination) program.

This full-time position is Monday-Friday; 9am-5pm, with flexibility


Duties & Responsibilities include, but are not limited to:

  • Maintain a caseload of 10-12 enrolled members and their families.
  • Use a strength-based and client-centered approach to build rapport and establish a therapeutic relationship with the enrolled member and their caregivers.
  • Develop a Care Team for each enrolled member, guided by member and caregiver input.
  • Identify and build on the strengths and culture of the member, family, and support system to create an individualized and integrated wraparound plan of care.
  • Develop an individualized Crisis and Safety plan with all required components.
  • Facilitate Child and Family Team Meetings (CFTM) that create a safe, effective, and supportive team environment and effectively meet the needs of the youth and caregivers.
  • Remain neutral and focused on the progress toward the vision while maintaining relationships with the member, family, helping professionals, and informal supports.
  • Connect the member and family to resources and ensure all parts of the plan are implemented promptly.
  • Teach and support the member and family to learn and use necessary skills to develop their own plans, access their own resources, and to be as independent as possible.
  • Understand and employ the 10 Principles of High-Fidelity Wraparound when carrying out positional duties and responsibilities.
  • Ensure all the requisite number of billable health home core services and face-to-face contacts are made during the month for all enrolled members.
  • Document essential components of the Wraparound planning process in the member’s electronic health record.
  • Comply with timelines for the completion of the assessments, development of the member’s Plan of Care and the scheduling of the initial and ongoing interdisciplinary team meetings.
  • Conduct comprehensive discharge planning with all members exiting the program.
  • Participate in monthly Quality Assurance meetings.
  • Develop a Safety and Crisis Plan, with member and family input.
  • Maintain and carry-out responsibilities set forth in the member’s Plan of Care, which may include making referrals, monitoring access to needed health care providers, linkage to social and community support providers, coordination amongst the interdisciplinary care team, and advocacy; monitors the completion of assigned tasks and goals.
  • Provide transitional care and support when a member is discharged from an Emergency Department or Inpatient setting, to ensure continuity of care.
  • Document the delivery of billable Health Home core services and all other case-related contacts in the contracted health home’s EHR.
  • Complete all mandated training as set forth by DOH, the contracted Health Home, and the MCOs.
  • Complete all regulatory requirements of the NYS Health Home Care Management Program (Community Outreach & Care Coordination).
  • Additional position-related duties and responsibilities as assigned by the Supervisor of Community Outreach & Care Coordination Program.

Qualifications:

  • Bachelor’s degree in psychology, social work, counseling, sociology, or other human service related field, required;
  • Minimum of 2 years’ experience in the human services field (child welfare, mental health, substance abuse and physical health).
  • Local travel is required for this position; must be able to meet the minimum requirements of our agency’s insurance policy for authorized drivers; which includes a minimum age of 21.
  • Must have a clean and valid NYS driver’s license; be actively licensed for at least one year and have a reliable (fully insured) vehicle for transporting clients (to community resources, meetings, etc.) - with proof of 100/300 liability insurance coverage.
  • Ability to work flexible hours to accommodate client schedules, which may include evening hours.
  • This position does have regular and substantial unsupervised or unrestricted contact with individuals receiving services and is required by law to be fingerprinted for a criminal history record check under DOH (NYS Department of Health).
  • Must possess the ability to make sound, independent decisions when circumstances are warranted.
  • Ability to pass the test to become a certified CANS assessor.
  • Previous experience working in a human services agency is a plus
  • Proficiency in various computer applications: Microsoft Office Suite (Word, Excel, PowerPoint), Google Business Workspace Suite (Gmail, Google Docs, Meet, Sheets, etc.); and secure Internet practices.
  • Must be willing and able to work in close proximity with the clients we serve.
  • Ability to pass required pre-employment background checks, to include fingerprinting, is required.

Essential Functions:

  • Ability to write and understand required documentation, including client progress notes and treatment plans; and to carry out written and electronic documentation required by St. Catherine’s policy and as required by regulating agencies that have oversight responsibility for St. Catherine’s programs.
  • Ability and willingness to maintain up to date information regarding standards and mandates set by St. Catherine’s Center for Children, the New York State Education Department, the New York State Office of Mental Health, the New York State Office of Child and Family Services, and other regulatory bodies.
  • The ability to legally operate a motor vehicle and to make home visits to member homes and other on-site contacts with referring agencies, collaborative agencies, foster homes, Campus Services sites, etc.
  • Flexibility to work hours outside of regularly scheduled work hours, within reason, if necessary.
  • Ability to work collaboratively with families, through a family-focused approach, welcoming them as part of the Campus Services Community.
  • The employee frequently is required to stand, walk and sit. Must be able to sit for long periods of time; must occasionally lift and/or move up to 25 pounds.
  • Certain deadlines and unanticipated developments may require work during evenings, weekends.
