Position Summary:

The HIV Outreach/Testing/Linkage Specialist (HIV/OTLS) will provide HIV testing, outreach, education, and linkage to services both within HCCH and to/with other collaborative agencies.

The HIV/OTLS will also provide patient care and case management services under the direct supervision of the TCE-HIV Coordinator and/or BH Director.

Responsibilities:

  • Provide HIV and other testing, counseling, education, and referral services to patients of HCCH and to the general public through community events, outreach, and other collaborative agencies.
  • Provide prevention interventions, education, and resources relative to sexually transmitted infections within HCCH and through community events, outreach, and off-site presentations.
  • Provide necessary support to physicians, nurse practitioners, physician assistants, therapists, behavioral health (BH) providers, and support staff engaged in treating HIV patients at HCCH.
  • Provide accurate HIV/AIDS (and other testing) data collection, tracking, and reporting within HCCH and to required health agencies (Health Dept, CDC, HRSA, etc.)
  • Accurately record patient health history information and vital signs into electronic record as directed. (HCCH Intergy/EHR, HRSA/CSAT).
  • Assist HIV Coordinator with procedures, referrals, and patient contacting as needed/required.
  • Educate patients regarding diagnosis, medications, treatments, and testing.
  • Schedule follow-up appointments for patients seen on day of service.
  • Obtain pertinent health information (Releases, consents, etc.)
  • Provides and arranges referrals to appropriate service providers within the agency and externally to ensure continuity of care and compliance to funding source requirements.
  • Advocates for the mission of the organization, promotes the vision and direction of the agency’s values, and delivers services with safety, respect, and effectiveness.
  • Follows all policies & procedures of the organization.
  • Performs other duties as assigned

Requirements:

  • HIV/AIDS 500/501 Certified (Basics/Prevention of Counseling, Testing, and Linkage) (Preferred)
  • At least one-year experience in Human Services field providing outreach and supportive case management services to at-risk, indigent, and/or homeless population (advocacy, housing placement, etc.), preferably working with HIV+ population and/or population(s) at high risk for HIV infection.
  • Comfortable working with LGBTQ+ population and settings where LGTBQ+ population frequents.

Knowledge, Skills and Abilities:

  • Must be able to work effectively with populations that have experienced vulnerability, trauma, economic poverty, incarceration, substance use, development delays, serious mental illness, traumatic brain injuries, literacy issues, and/or other conditions or situations that have impacted housing stability
  • Ability to work in a fast-paced environment
  • Ability to be flexible and able to multi-task
  • Must be solution-focused and organized in achieving objectives with all service users
  • Must be able to motivate and organize self to complete tasks with service users, often in non-therapeutic environments
  • Ability to maintain confidentiality and follow all federal and state HIPAA laws and regulations
  • Ability to handle stress and high pace of crisis intervention and follow protocols for safety and security
  • Excellent oral and written communication skills, including use of phones, in-person assistance, computers, fax, and other office machinery
  • Computer proficient (Microsoft Office suite) and experience with EHR (Electronic Health Record) documentation preferred
  • Experience with HMIS (Homeless Management Information System) preferred.
