Qualifications:

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Minimum of three (3) years of nursing experience.
  • Current and valid LPN license in the state of Illinois.
  • Current CPR certification.
  • Strong clinical and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Reliable transportation and a valid driver's license.

Responsibilities:

  • Provide direct patient care as defined in the state Nurse Practice Act.
  • Implement the plan of care initiated by the registered nurse (RN).
  • Provide accurate and timely documentation consistent with the plan of care.
  • Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Participate in the coordination of home health services, appropriately reporting identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the RN and/or Clinical Supervisor.
  • Use and/or prepare equipment and supplies effectively and efficiently, adhering to aseptic technique.
  • Participate in personal and professional growth and development.
  • Perform other duties as assigned by the RN.
  • Provide healthcare instructions to the patient as appropriate per assessment and plan of care.
