POSITION

Home Health Administrative Certified Nursing Assistant


SUMMARY

Home Health Administrative Certified Nursing Assistant is a hybrid role with nursing assistant duties including monitoring the health condition of people with disabilities and chronic illness, helping them with daily living activities within the community, and administrative duties such as routine clerical and organizational tasks.


QUALIFICATIONS

  • Excellent verbal and written communication and interpersonal skills

  • Commitment to providing compassionate and efficient person-centered care

  • Active Maryland Certified Nursing Assistant (CNA) license

  • Active CPR / First Aid certification

  • Active driver’s license, auto insurance, reliable transportation, and the ability to travel to patients' homes within the assigned geographic area

  • Clear criminal background check

  • Negative Hepatitis B test

  • Negative tuberculosis test within the last 12 months or a clear chest x-ray within the last 5 years

  • Computer proficiency

  • Three professional/personal references


PREFERRED

  • Two (2) years of CNA experience.

  • Comfortable in an environment where participants cannot perform basic life duties.

  • Ability to prioritize multiple deadlines in a fast-paced environment.

  • Ability to establish rapport with participants, families, and interdisciplinary teams.

  • Ability to work independently with minimal supervision and collaborate within a team.

  • Understanding the basic concepts of personal care, housekeeping, and other care areas.

  • Clean medical physical within 12 months.


NURSING RESPONSIBILITIES

  • Provide direct patient care according to care plans and physician orders.

  • Conduct assessments and assist patients with activities of daily living (ADLs) and independent activities of daily living (IADLs).

  • Monitor patients' health status, documenting and communicating findings to the healthcare team.

  • Identify and address safety hazards and environmental factors in participants’ homes to ensure a safe and supportive care environment.

  • Document patient care activities in electronic health records (EHR) systems.

  • Participate in patient case conferences.

  • Participate in continuing education programs, including at least 12 in-service courses within 12 months, to enhance clinical knowledge and skills.

  • Demonstrate the safe, effective, and efficient use of equipment and supplies.

  • Participate in extensive physical activity such as bending, lifting, and standing regularly.

  • Work for an extended period while standing and being involved in physical activity.

  • Heavy lifting.


ADMINISTRATIVE RESPONSIBILITIES

  • Communicate with participants, family members, healthcare team, and social support with compassion and respect.

  • Obtain authorizations for covered services.

  • Schedule and coordinate appointments.

  • Maintain accurate and up-to-date records in an electronic health record (EHR) system.

  • Maintain home health supply inventory through regular audits.

  • Work as a team with healthcare providers, case managers, and referral sources to provide information about home health services, eligibility criteria, and care options.

  • Participate in meetings and continuing education to ensure compliance with regulatory requirements, accreditation standards, and organizational policies.


WORKING ENVIRONMENT

Participant homes (including travel to/from) and office settings.

Supervisor: Director of Clinical Services/Clinical Manager, Physicians, Registered Nurses,

Physical Therapists, and Occupational Therapists.

Subordinate: This is not a supervisory role.
