Job DescriptionJob Description

Responsibilities:

  • Responsible for Veteran Surge Efforts in SPA 5 and/or SPA 3 that consist of case conferencing meetings with veteran providers, coordination of monthly outreach with local providers and the prioritization of highly vulnerable clients as housing resources expand.
  • Works in concert with SPA 5 & 3 CES lead agency and the Los Angeles Homeless Service Authority (LAHSA) to manage Veteran’s By-Name List.
  • Coordinates with housing navigation teams to facilitate individuals experiencing homelessness with permanent housing.
  • Administers Vulnerability Index-Service Prioritization Decision Assistance Tool (VI-SPDAT) to homeless contacts established through outreach efforts.
  • Maintains collaborative and positive relationships with partner agencies that include the Veterans Administration and other community partners.
  • Attend staff trainings and department meetings as needed and other duties as assigned.

Required Skills

  • Two Year of experience working in a related position that involved the coordinated entry process, assessment, reporting and case conferencing.
  • Prior experience working with individuals living with mental illnesses and a history of chronic homelessness
  • Ability to coordinate with multiple agencies and service providers to facilitate assessment for individuals experiencing homelessness.
  • Proficient with Service Prioritization Decision Assistance Tool (SPDAT) assessments and triage tools.
  • Possess excellent organization skill, initiative and follow through.
  • Be detail oriented, able to set priorities, meet deadlines, maintain a professional attitude, conduct and appearance at all times.
  • Proficient in office software including:  Word, Excel, Outlook, HMIS and Internet use.

Education & Other Requirements

  • Bachelor’s degree preferred, work experience can substitute.
  • Valid Driver’s License, reliable transportation, current vehicle registration, proof of insurance and the ability to be insured by New Directions.
  • Ability to successful complete background check, drug test and Annual TB test.
