Outreach Coordinator

Responsibilities:
- Responsible for Veteran Surge Efforts in SPA 5 and/or SPA 3 that consist of case conferencing meetings with veteran providers, coordination of monthly outreach with local providers and the prioritization of highly vulnerable clients as housing resources expand.
- Works in concert with SPA 5 & 3 CES lead agency and the Los Angeles Homeless Service Authority (LAHSA) to manage Veteran’s By-Name List.
- Coordinates with housing navigation teams to facilitate individuals experiencing homelessness with permanent housing.
- Administers Vulnerability Index-Service Prioritization Decision Assistance Tool (VI-SPDAT) to homeless contacts established through outreach efforts.
- Maintains collaborative and positive relationships with partner agencies that include the Veterans Administration and other community partners.
- Attend staff trainings and department meetings as needed and other duties as assigned.
Required Skills
- Two Year of experience working in a related position that involved the coordinated entry process, assessment, reporting and case conferencing.
- Prior experience working with individuals living with mental illnesses and a history of chronic homelessness
- Ability to coordinate with multiple agencies and service providers to facilitate assessment for individuals experiencing homelessness.
- Proficient with Service Prioritization Decision Assistance Tool (SPDAT) assessments and triage tools.
- Possess excellent organization skill, initiative and follow through.
- Be detail oriented, able to set priorities, meet deadlines, maintain a professional attitude, conduct and appearance at all times.
- Proficient in office software including: Word, Excel, Outlook, HMIS and Internet use.
Education & Other Requirements
- Bachelor’s degree preferred, work experience can substitute.
- Valid Driver’s License, reliable transportation, current vehicle registration, proof of insurance and the ability to be insured by New Directions.
- Ability to successful complete background check, drug test and Annual TB test.
