Overview: We are seeking a dedicated and compassionate DDD (Division of Developmental Disabilities) Home Care Coordinator to join our team. This role is pivotal in ensuring the coordination and delivery of quality home care services to individuals with developmental disabilities. The ideal candidate will possess excellent organizational skills, a deep understanding of DDD regulations and policies, knowledge of sales and marketing as well as a passion for enhancing the lives of clients through personalized care solutions.
Responsibilities:
- Coordinate all aspects of home care services for clients with developmental disabilities, ensuring compliance with DDD regulations and guidelines.
- Conduct initial assessments and develop individualized care plans based on client needs, preferences, and DDD requirements.
- Collaborate with clients, families, caregivers, and healthcare providers to ensure comprehensive and holistic care delivery.
- Monitor and evaluate the effectiveness of care plans, making adjustments as necessary to optimize client outcomes.
- Serve as the primary point of contact for clients and their families, addressing inquiries, concerns, and emergencies promptly and professionally.
- Maintain accurate and up-to-date documentation of client assessments, care plans, progress notes, and communication logs.
- Coordinate interdisciplinary team meetings to review client progress, discuss challenges, and develop strategies for improving care delivery.
- Advocate for clients’ rights and access to community resources, ensuring they receive the support needed to enhance their quality of life.
- Stay informed about current trends and best practices in DDD home care services, participating in ongoing training and professional development activities.
Qualifications:
- High school diploma; Bachelors (preferred).
- Article 9
- CPR and First aid
- Fingerprint clearance card
- Experience in coordinating home care services, preferably with individuals with developmental disabilities.
- knowledge of DDD regulations, policies, and procedures.
- Exceptional organizational and time management skills with the ability to prioritize tasks effectively.
- Excellent interpersonal and communication skills, with the ability to build rapport and collaborate effectively with diverse stakeholders.
- Compassionate and patient-centered approach to care, with a commitment to advocating for clients’ needs and preferences.
- Proficiency in using electronic health records (EHR) and other relevant software applications.
- Experience in sales, with a proven track record of achieving targets and building client relationships.
- Proven ability to market services effectively, drive sales, and achieve business growth targets
