This role is pivotal in ensuring the coordination and delivery of quality home care services to individuals with developmental disabilities. The ideal candidate will possess excellent organizational skills, a deep understanding of DDD regulations and policies, knowledge of sales and marketing as well as a passion for enhancing the lives of clients through personalized care solutions.

Responsibilities:

  • Coordinate all aspects of home care services for clients with developmental disabilities, ensuring compliance with DDD regulations and guidelines.
  • Conduct initial assessments and develop individualized care plans based on client needs, preferences, and DDD requirements.
  • Collaborate with clients, families, caregivers, and healthcare providers to ensure comprehensive and holistic care delivery.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as necessary to optimize client outcomes.
  • Serve as the primary point of contact for clients and their families, addressing inquiries, concerns, and emergencies promptly and professionally.
  • Maintain accurate and up-to-date documentation of client assessments, care plans, progress notes, and communication logs.
  • Coordinate interdisciplinary team meetings to review client progress, discuss challenges, and develop strategies for improving care delivery.
  • Advocate for clients’ rights and access to community resources, ensuring they receive the support needed to enhance their quality of life.
  • Stay informed about current trends and best practices in DDD home care services, participating in ongoing training and professional development activities.

Qualifications:

  • High school diploma; Bachelors (preferred).
  • Article 9
  • CPR and First aid
  • Fingerprint clearance card
  • Experience in coordinating home care services, preferably with individuals with developmental disabilities.
  • knowledge of DDD regulations, policies, and procedures.
  • Exceptional organizational and time management skills with the ability to prioritize tasks effectively.
  • Excellent interpersonal and communication skills, with the ability to build rapport and collaborate effectively with diverse stakeholders.
  • Compassionate and patient-centered approach to care, with a commitment to advocating for clients’ needs and preferences.
  • Proficiency in using electronic health records (EHR) and other relevant software applications.
  • Experience in sales, with a proven track record of achieving targets and building client relationships.
  • Proven ability to market services effectively, drive sales, and achieve business growth targets
