We are seeking several Nurses (RN Or LPN) to join our home health team You will be responsible for the treatment and certification of assigned patients.
Responsibilities:
- Administer nursing care to ill, injured, or disabled patients and re-certify patients for services
- Monitor and report changes in patient symptoms or behavior
- Educate patients about health maintenance and disease prevention
- Maintain accurate patient medical records time reporting
- Use excellence as your yardstick
Qualifications:
- Previous Nursing experience in home health care is a plus
- Ability to build rapport with patients and good communication skills
- Current Ohio Nursing license
- 24-36 hours per week, Monday, Tuesday and Wednesday (LPN) for a specified patient
- Also home visits with multiple patients (RN or LPN)
