Summary:
- Perform outreach based on assignments received through various methods, including outbound calls to various providers, members, vendors, or other outside contacts to increase health behavior participation and drive positive health outcomes for our members.
- Make direct calls to members using effective engagement strategies to respectfully encourage members to utilize available benefits in support of member health behavior participation.
- Perform at the expected level of service to enhance customer interaction and experience.
- Coach, educate and guide members in health program participation and preventive care tactics.
- Maintain member confidentiality per HIPAA compliance guidelines.
- Provide cross functional support to varied internal stakeholders.
- Responsible for maintaining reports, tracking and/or data analysis as assigned or as needed.
- Provide regular communication to leadership and other team members on the results of member and/or provider engagements including maintaining clear, accurate, concise documentation of communications with members/providers.
- May support other team members through referrals, mailbox management, work assignment and relevant documentation.
- Performs other tasks as assigned by leadership.
- Understanding of electronic health record (EHR) systems. Willingness to learn new concepts, growth mentality, and ability to think outside the box. Understanding of DoH flags to be able to provide creative solutions to challenges that may be experienced by members. Experience with care coordination, HEDIS/closing care gaps, or medical office support.
Preference Given To:
- Understanding of scheduling systems. 1-2 years’ experience in healthcare and/or customer service, Ability to adapt to change and be comfortable in working in a fast-paced environment.
What You'll Bring (Hiring Requirements)
- High school diploma or GED. Comfortable with technology such as the Microsoft Office Suite of products and technology enabling contact with members.
