Job DescriptionJob Description

This position is located in a Naperville SNF and is FT M-F. It requires call from the one facility only from 5-9pm. We are looking for an NP who has strong clinical skills, is a good communicator and shows initiative. Acute care or LTC experience preferred. We require a minimum commitment of one year, but strongly desire a long-term commitment with loyal and dedicated employees who can partner with us. Highly Competitive Compensation.

Responsibilities:

· Examines, diagnoses and treats newly admitted patients and existing patients in EMR

· Collaborates and communicates with Primary Physician and facility nursing staff

· Performs comprehensive physical examinations

· Orders, interprets, and evaluates diagnostic tests

· Prescribes or recommends medications or other forms of treatment

· Collaborates and communicates with healthcare professionals, patient and family of patient

· Records physical findings in EMR/Patients Records within specific time frame

· Submits and reports all scheduling, payroll and billing in a timely manner

· Effectively communicates with colleagues and facility staff for all facility problems

· Attends bi-monthly meetings

 

Qualifications:

· Required education:Master’s Degree

· Required license or certification:Nurse Practitioner certification, IL APN and CS lic, DEA

· Required:1+ years of experience

· Preferred:An acute care background or LTC experience
