Position Overview: Are you an enthusiastic and skilled Registered Nurse (RN) looking to make a meaningful impact in the field of home health care? Join our vibrant team in Macon, GA, and contribute to delivering top-notch personalized care to patients within the comfort of their homes. If you're an active RN with a year of recent home health experience, possess BLS Certification from the American Heart Association, and are proficient in OASIS documentation and the Homecare Homebase (HCHB) system, we invite you to be a part of our team

Key Requirements:

  • Active RN Licensure
  • Minimum of one year of recent home health experience

Responsibilities: As a valued member of our team, you will:

  • Collaborate closely with a diverse group of healthcare professionals to design and implement individualized home care plans.
  • Perform comprehensive patient assessments, laying the foundation for uniquely tailored care strategies.
  • Administer medications, offer wound care, and provide expert nursing interventions.
  • Maintain a vigilant eye on patient progress, facilitating seamless communication within interdisciplinary teams.

  • Duration: 13-week contract position within the realm of home health care excellence.
