Job DescriptionJob Description

The Evergreen Mentorship Program is seeking assessors to be the first point of contact for incoming clients of the organization. Assessors commit to time blocks according to their schedule and availability, with these time blocks remaining the same from week to week. Each assessment takes roughly 45-60 minutes to complete via phone call. Responsibilities of this position include, but are not limited to, the following:

 

  • Conduct an intake assessment with each client scheduled during your assigned block of time

  • Evaluate clients’ current and past biopsychosocial condition and provide a clinical diagnosis according to DSM-5 standards

  • Provide a treatment plan for the client according to their assessment and diagnosis

  • Clearly and concisely explain services offered by the organization

  • Obtain client consent for treatment and participation in the program

  • Input client assessment, diagnosis, and treatment plan into the organization’s EHR system in a timely manner (48-72 hours)

  • Maintain current and continued knowledge of DSM-5 diagnoses

  • Attend all required meetings and trainings

  • Maintain consistent and reliable attendance

  • Exhibit positivity, adaptability, and excellent communication skills both written and verbal

  • Present the organization in the most positive light with all internal and external contacts

 

Minimum qualifications for the position:

 

  • Up to date knowledge of DSM-5 diagnoses

  • Excellent communication skills both written and verbal

  • Access to a reliable computer and internet connection

  • Ability to maintain high levels of organization, attention to detail, and confidentiality

  • Proven history of experience working with BIPOC and LGBTQ+ populations

  • Advanced computer competency for creating and maintaining client records

  • Licensed in the state of Ohio as a completely independent licensed Social Worker, Counselor, etc.

 

Additional Information:

 

  • The rate of pay for this position is $75.00 per completed assessment - this includes a completed intake assessment, diagnosis, treatment plan, and entry into EHR system

  • This position is entirely virtual and remote, with all assessments being conducted via phone call (some hearing impaired clients may also request an assessment to be done via text message)

  • 1099 contractor position
